Outpatient CT Referral

Thank you for your patient referral to VSx Veterinary Veterinary Surgery of Indianapolis. Please complete the Outpatient CT Referral form to provide as many details as you can about your patient to assist us in best serving them and your client.

Note:

• Outpatient CT Referral is for imaging purposes only and results are sent directly to rDVM for review and client communication. A Consult appointment is needed if you wish for a surgeon to review the CT findings to aid in interpretation and/or provide input on diagnostic or therapy plans and/or be involved in client discussion.

• A request for a contrast study requires rDVM to provide pre-anesthetic bloodwork within 6 weeks of the scan.

• Referral requests for CT imaging must come from a referring veterinarian.

• For patients with higher anesthetic risks, we may request they schedule a consult first.

After your patient’s CT, we will send the interpretation to the noted clinic email. Do not hesitate to reach out if you have any follow up questions.
