Thank you for your patient referral to VSx Veterinary Veterinary Surgery of Chicago. Please complete the Outpatient CT Referral form to provide as many details as you can about your patient to assist us in best serving them and your client.

Note: For patients with higher anesthetic risks, we may request they schedule a consult first.

After your patient’s CT, we will send the interpretation to the noted clinic email. Do not hesitate to reach out if you have any follow up questions.
